Georgia Department of Public Health requires all students born on or after January 2002 and entering or transferring in to 7th grade have proof of a whooping cough booster and meningococcal shot. In addition, effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4), unless their initial dose was administered on or after their 16th birthday.
Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord. Meningitis can cause shock, coma, and death within hours of the first symptoms.
If your teen has not been vaccinated against meningococcal disease, please call your pediatrician or family doctor for more information. If your child does not have insurance or your insurance does not cover these vaccines please call our local health department and ask about low cost vaccine options.
It is a state law that students who attend school in Georgia have an up to date immunization form (Form 3231) on file. You will be required to provide the school with proof of vaccination within the first 30 days of attending or your child may be excluded from classes. If your child does not receive immunizations based on religious beliefs, you must provide a signed and notarized affidavit, and that form must be on file in their permanent record.
